All parts of the Form R must be submitted via TIS Self Service (TSS) (except Dentistry and Foundation) – old style forms (PDF of Word documents) will not be accepted.
The Form R is a mandatory GMC requirement for all ARCPs as part of their Revalidation process. A Form R (Part B) must be submitted prior to every ARCP.
A Form R (Part A and Part B) is also required at the start of training as part of the registration process.
TIS Self Service enables a resident doctor to log in and see some data NHS England hold. It will also enable the completion of the Form R process. Over time we will add data and functionality which will include response to feedback received from resident doctors.
TSS is used by all regions of NHS England. If you have previously registered with TSS with another NHS England region, you do not have to sign-up again, and you should log-in using your previous log-in details.
To sign-up you must use the primary email address that NHS England have for you. If you would like to update your email address, please contact the relevant specialty school before following the registration process.
Once completed, please save a copy of your Form R in your e-portfolio.
