CT1/2 Training

The Royal College of Anaesthetists has laid down strict criteria regarding the content and assessment of the CT1/2 curriculum details of which are available from any College Tutor or the RCA website www.rcoa.ac.uk. They include Introductory Training, followed by experience of Anaesthesia for a wide range of surgical specialties including General Surgery, Gynaecology, Urology, ENT, Maxillo-facial Surgery, Ophthalmology, Orthopaedics, Plastic Surgery, Day Case Surgery and Obstetrics (normally in the second year). Trainees should also be given training in the Management of Acute Pain, Resuscitation and Intensive Care.

Anaesthetic trainees are expected to be ALS providers.  Candidates whose status has lapsed will be expected to renew it.  Local tuition [by the individual hospital resuscitation trainers] is also required.

The Southern Hospital Group currently includes the South Manchester University Hospital  (Wythenshawe), Tameside General Hospital, Royal Oldham Hospital and Stepping Hill Hospital, Stockport.  All hospitals have a non-smoking policy.

The Southern Hospital Group has combined resources to offer an individualised comprehensive training to the CT1/2s within their auspices.  This training will comprise experience in all the areas outlined in the CT1/2 curriculum.  A centralised structured tutorial programme complements it.  There are introductory tutorials held at Stepping Hill Hospital for the trainees in their first 3 months.  Thereafter, tutorials aimed at the primary FRCA take place at Wythenshawe Hospital.  Any trainees who have passed Primary FRCA can take Final tutorials also held at Wythenshawe Hospital.  CT1/2s will be expected to keep a Log Book and to participate in regular appraisal.  The scheme will also be regularly evaluated.

Successful candidates will be offered a starting post in a specific hospital and will initially spend 1 year there.  Following an appraisal of the trainees' educational needs subsequent training will be offered within the group to provide a comprehensive training. Usually the 2nd year will also be spent in one hospital. The primary purpose of CT1/2 training is to prepare trainees for competitive entry to ST 3-7 posts in Anaesthesia in the Northwest Region.

Appointments will all be subject to satisfactory progress.  Attendance at the Regional Primary FRCA day release course is encouraged.   In each hospital the College Tutor will act as Educational Supervisor during the CT1/2s’ stay. 

Further information or clarification can be obtained from the Programme Director or any of the College Tutors within the group. Visiting the hospitals is not generally encouraged but can be discussed if a candidate feels necessary.

The Hospital

South Manchester University Hospitals NHS Trust was formed in 1994 and comprised Withington and Wythenshawe Hospitals.  A PFI construction was completed at Wythenshawe in September 2001, and now all acute services are on this one site. There is a tertiary referral Intensive Care Unit, Burns and Plastics and a Cardiothoracic Unit. The all-new Withington Community Hospital has been recently opened; some outpatient anaesthesia is likely to be provided on this site, but trainees are unlikely to be involved.

The Department

The Anaesthetic department provides anaesthetic services for General and Vascular Surgery, Dental and Maxillofacial Surgery, Urology and Orthopaedics, Day Case Surgery, Orthopaedic, ENT, and Gynaecology.  The Obstetric unit undertakes 3,500 deliveries.  There are Acute Pain and Chronic Services.  Anaesthetic services are also provided for Christie Hospital about 6 km distance (with experience in radiotherapy, chemotherapy and surgical oncology), but CT1/2s are rarely involved in this.

Training Opportunities

Each new CT1 will be supernumerary for at least 3 months.  They will attend operating lists with Consultants and other senior staff.  After a short time they will be allocated to 'shadow' a more senior trainee for evening and weekend emergency work.  According to individual aptitude, they will be introduced to independent practice over the next 3 months or more following satisfactory assessments. Apart from the ICU module, taken in the second six months, the CT1 will remain supernumerary in regard to on-call during the first year

Educational Facilities

There is secretarial help and office accommodation for Consultants and trainees. Computing facilities with logbook programmes are available for trainees.  The hospital has an active postgraduate department, a well-equipped medical library and appropriate reference books and journals are available in the department.  There are seminar rooms in the department with presentation facilities which trainees are expected to use.  Regular trainee meetings with consultant support and general medical meetings occur, which all trainees are encouraged to attend.   The Intensive Care Unit trainees have a structured course of tutorials during the module.  The College Tutors will arrange appraisal meetings with each CT; formal assessment of clinical competence will take place from time to time.  The RCA requires an average of 3 teaching (accompanied) lists per week.  We expect to exceed this target.

Audit

There are bi-monthly audit meetings.  Involvement in audit projects is expected when trainees are not preparing for exams.  There are facilities for research.

The Hospital

Tameside General Hospital is located near Ashton-under-Lyne, about 7 miles east of Manchester City Centre.  It provides a wide range of District General Specialties.  There is an Intensive Care Unit.

The Department

The Department provides office space, secretarial support and a seminar room with presentation facilities.  There are regular departmental and audit meetings, with an annual guest lecture.  There is a computer with Internet access.

Educational Opportunities

There is a postgraduate medical centre, with an active teaching programme in which residents are expected to participate.  There is a comprehensive library with facilities for CD-Rom literature searches and video tape cassette learning.  There is an average minimum of 3 accompanied (teaching) operating lists per week.

The Hospitals

Stepping Hill is a major District General Hospital located to the South East of Manchester.  There is a full range of DGH services including eye surgery and an obstetric unit (4000 deliveries per annum) and an Intensive Care Unit (7 beds).  There is an Acute Pain Service and a Chronic Pain Clinic.

The Department

The Anaesthetic department provides office space, a seminar room with presentation facilities and communal areas.  The training areas are equipped with a multimedia computer and library facilities.  There are weekly departmental meetings and audit meeting in which participation is encouraged.  There is also a well-equipped postgraduate centre.

The Hospital

The Royal Oldham Hospital is a largely new hospital of 850 beds.  There is a 6 bed Intensive Care Unit and a 6 bed High Dependency Unit.  It provides a full range of DGH services including vascular surgery, eye surgery and a maternity unit with a 24 hour obstetric epidural service.  The unit is well known for an interest in regional anaesthesia and analgesia.  TIVA is widely practised.  There is an excellent Acute Pain Service and a Chronic Pain Clinic.

The Department

The Anaesthetic department has office space, secretarial support and a seminar room.  Full computing facilities are available.  There is a regular journal club and monthly audit meetings.  There is a new Postgraduate Medical centre with a well equipped library and full time librarian. Teaching lists are maintained within recommended levels.

On-Call

CT1s are resident first on-call.  All hospitals are actively committed to New Deal compliance, as per national agreements.  Currently audits are underway, and the rota systems are in a period of change.  The precise rota differs between the hospitals.  For exact details of payments please seek advice from individual Hospitals.

Administration

The junior doctor accepts that he/she will perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate consultant, in consultation where practicable with his colleagues both senior and junior.  It has been agreed between the professions and the department that while juniors accept that they will perform such duties the Secretary of State stresses that additional commitments arising under the sub-section are exceptional and in particular that juniors should not be required to undertake work of this kind for prolonged periods or on a regular basis.

Teaching

Tutorials are centralised and for trainees of all the school’s hospitals. Those for beginner CT1s take place at Stepping Hill.  Trainees preparing for the Primary examination have tutorials held at Wythenshawe (UHSM) Anaesthetic Department. CTs who have passed this hurdle may attend Finals tutorials, also at Wythenshawe.
Day release attendance at appropriate North West Fellowship courses is also expected.
Attendance at the Anaesthetic Section of the Manchester Medical Society and other local specialty meetings is also encouraged.

Research and Audit

Opportunities exist for participation in projects; at least one audit per year is expected of the School’s CTs.
