If you answer ‘yes’ to any of the Fitness to Practise questions on your Specialty Training application form you will need to complete a Fitness to Practice Declaration form. This form can be downloaded from the Oriel resource bank.
Please return all completed declaration forms to our confidential fitness to practice email address before the closing date of the application round that you are submitting an application in: england.fitnesstopractise.nw@nhs.net
